How to Refer a Patient
Referring a patient to MKGenetix is straightforward. Please follow these steps:
- Complete the Referral Form: Complete our Referral Form or use your own referral form and submit it to MKGenetix through one of the following methods:
- Email: info@mkgenetix.com
- Online Submission: Register for an account with us (no charge) and then use the Referral Submission Portal.
- Complete and fax our Referral Form to 877-497-0459
- Include Supporting Documents: Attach any relevant medical records, test results, and family history information to provide a comprehensive view of the patient’s case.
- Schedule an Appointment: Once the referral is received, our office will contact the patient directly to schedule an appointment. Providers can also call our office to schedule on behalf of their patients.
- Follow-Up: After the counseling session, a detailed summary and recommendations will be sent to the referring provider to ensure seamless continuity of care.
